Doctor's requisition order (unless you are requiring service for a specialty kit)
Note: For specialty kits, we strongly recommend contacting us first before booking an appointment.
Health Insurance information
Photo ID to verify your identity
Convenience fee for appointment booking (we do not currently accept insurance)
Some insurance providers may reimburse you but that is totally out of our control and is a decision solely made by insurance. We will however provide you with proof of service provided and any other documentation should you need them to claim your reimbursement.

Complete your booking (Payment is required at time of booking)
Upload your requisition, if applicable (** This is required for all "Routine Blood Draws")
AFTER BOOKING:
When we receive your booking, we will contact you via email or phone to go over important details and confirm your appointment.
A separate email will be sent to you to request your doctor's requisition if we have not received it (a doctor's requisition is required for routine blood tests).
When you receive this email, please reply with a copy of the requisition as soon as possible.
Once your booking is confirmed, a member of our team will be assigned to you.
They will go over your test order to prepare in advance for your blood draw. They may contact if some information is missing, needed or for other reasons related to your booking.
They will call or text you a day before and/or on the morning of the appointment to let you know they will be coming.
On the day of your appointment, your assigned phlebotomist will travel to your location to draw your blood.
Please note: the appointment scheduling time is not a perfect time system. We consider it a window rather than an exact time slot as we cannot always control factors surrounding the travel nature of our services. While we will try our very best to meet the exact time booked, please allow a 15-30 mins window of arrival. The phlebotomist may arrive 10-15 mins earlier or later than the booked time (for example: if you book a 9:40am appointment, the arrival window is 9:25am - 9:55am). Your assigned phlebotomist will however keep you updated on their ETA.
The blood draw process will typically take up to 10 mins for the most basic draw.
Your assigned phlebotomist will process and drop off samples at the laboratory.
The lab will run the tests and send the results to your ordering provider. (Please note: test results are not sent to us)
To view your results, contact your provider who ordered the blood test.
For specialty kits, you may need to log into your online account or follow the instruction manual provided in your kit to view your results.
The lab will bill your insurance for the tests.
If self-pay, they are billed to the account you have on file or payment information must be provided to the phlebotomist to attach to your specimen. There may also be the option to make a payment online depending on the laboratory. If you are not sure about how payment for your lab tests are going to be covered please contact your ordering physician.
